To assist our customers and expedite our service to them we have provided consultation forms to be completed prior to your consultation appointment.

The cost of the hormone consult is $50.00 per 30-minute one-on-one session.  The cost of the consult is NOT covered by any insurance provider. 

• Included with the consult:

•     1.  The medication dosage recommendation and follow-up with your selected prescriber
•     2.  Clinical follow-up and adjustments during the first year of therapy
•     3.  Supplement consultation to facilitate optimal therapy goals
•     4.  Email and phone access to Bridgette for 1 year to answer questions or address additional      hormone related concerns.

The following consultation forms can be  printed and faxed, or dropped off to us at the pharmacy.  The HIPAA form for Females can also be filled out online and submitted directly to us by clicking on the submit button at the bottom of the form below.
